2018 Virginia Community Healthcare Association Annual Awards Nomination Period is March 1st – March 30th

If you wish to nominate someone for the Annual Awards, please review the categories below and complete this form by March 30th. Please note that only Member Organizations in “Good Standing” may submit nominations. 2018 Annual Awards Categories

New Category   Rod Manifold Young Professional Award
The focus of this award centers on the exceptional advancement of the goals and missions of the organization. Nominations for this award may only be made by the health center Executive Director or Chief Executive Officer. This award is presented to a recipient no older than the age of thirty-five at the time of nomination. 

The Rod Manifold Young Professional award highlights the accomplishments of an exceptional staff person of a Member Organization who has helped further the mission of providing high-quality, culturally-competent health services to underserved populations and supports the organization’s overall mission.


William E. Cole Award: Friend of the Association
The focus of this award centers on advocating for and championing the goals and mission of community health care. 

The William E. Cole Award recognizes an individual who has advocated for or aided community health care in areas such as: political advocacy; resource advocacy (i.e., public servants within legislative or administrative branches of local, state or federal government); public or private agencies (such as the National Association of Community Health Centers); or program/community development assistance (for example, long-standing technical assistance).


Volunteer of the Year
The focus of this award centers on the exceptional promotion of community health care.

The Volunteer of the Year Award recognizes an outstanding board member, unpaid staff member, or an individual other than board or unpaid staff who has served voluntarily to promote community health care.

 

 


Provider of the Year
The focus of this award centers on the provision of outstanding patient care.

The Provider of the Year Award recognizes an outstanding health care provider who has demonstrated dedication, expertise and concern for their patients in a compassionate and comprehensive manner. Provider is defined as a physician, dentist, pharmacist, behavioral health practitioner or advanced nurse practitioner, or physician assistants who may or may not be employed by an Association Member.

 

 


Nurse of the Year
The focus of this award centers on the provision of outstanding patient care.

The Nurse of the Year Award recognizes a nurse who has demonstrated dedication, expertise and concern to see that patients are afforded the best quality care and who has demonstrated outstanding quality in working with staff, patients, and family members.

 

 


Staff Member of the Year
The focus of this award centers on exceptional dedication to patients.

The Staff Member of the Year Award recognizes a non-medical employee of a health center who has demonstrated dedication and concern to see that patients are afforded the best quality care and who has demonstrated outstanding quality in working with staff, patients, and family members.

 

 


Team of the Year
The focus of this award centers on the exceptional advancement of the goals and missions of the organization.

The focus of this award centers on exceptional advancement of the goals and missions of the organization.
