2022 Annual Awards – Call for Nominations

Annual Awards Nomination Period is July 15 – August 15

VCHA will host its annual membership meeting and conference this fall at the Virginia Crossings Hotel & Conference Center in Glen Allen, Virginia, from September 28 – 30. At the conference, we will be recognizing individuals for their achievements during an awards ceremony. If you wish to nominate someone for the Annual Awards, please review the categories below and complete this form by August 15. Award Categories:

Volunteer of the Year
The focus of this award centers on the exceptional promotion of community health care.

The Volunteer of the Year Award recognizes an outstanding board member, unpaid staff member, or an individual other than board or unpaid staff who has served voluntarily to promote community health care.

 

Provider of the Year
The focus of this award centers on the provision of outstanding patient care.

The Provider of the Year Award recognizes an outstanding health care provider who has demonstrated dedication, expertise and concern for their patients in a compassionate and comprehensive manner. Provider is defined as a physician, dentist, pharmacist, behavioral health practitioner or advanced nurse practitioner, or physician assistants who may or may not be employed by an Association Member.

 

Nurse of the Year
The focus of this award centers on the provision of outstanding patient care.

The Nurse of the Year Award recognizes a nurse who has demonstrated dedication, expertise and concern to see that patients are afforded the best quality care and who has demonstrated outstanding quality in working with staff, patients, and family members.

 

Staff Member of the Year
The focus of this award centers on exceptional dedication to patients.

The Staff Member of the Year Award recognizes a non-medical employee of a health center who has demonstrated dedication and concern to see that patients are afforded the best quality care and who has demonstrated outstanding quality in working with staff, patients, and family members.

 

Team of the Year
The focus of this award centers on the exceptional advancement of the goals and missions of the organization.

The Team of the Year Award recognizes a team within a Member Organization that demonstrates outstanding synergy working together toward the common goals and missions of the organization. The focus of this award centers on goals and missions of the organization.

 

The Torchbearer Award
This award recognizes an individual responsible for significant advocacy promoting VCHA’s legislative and regulatory priorities. 

The Torchbearer Award recognizes individuals for their commitment to advocacy on behalf of the Association and Virginia’s Federally Qualified Health Centers and whose legislative and policy efforts have advanced healthcare progress in profoundly important ways.

 

The A1C Award
This award recognizes a member health center for outstanding success in lowering A1C rates among their patient population.

The A1C Award recognizes health centers for outstanding contributions in patient-oriented clinical outcomes research having significant impact on diabetes prevention and treatment.

 

The Special Recognition Award
The focus of this award centers on exceptional advancement of the goals and missions of the organization.

The Special Recognition Award recognizes an outstanding staff person of a Member Organization who is a non-provider or non-volunteer for their extraordinary efforts in support of the organization’s overall mission.

 

Other criteria for assessment of all nominees include:

  • Consistency of Involvement and Performance
  • Personal Character
  • Level and Scope of Involvement
