Patient Centered Medical Home (PCMH)
PCMH is a team-based health care delivery model which provides comprehensive and continuous medical care to patients with the goal of obtaining maximized health outcomes. It focuses on the triple aim of improving quality of care, increasing patient satisfaction, and reducing costs.
The Virginia Community Healthcare Association is committed to helping all Community Health Centers (CHCs) across Virginia achieve Patient Centered Medical Home (PCMH) transformation and recognition.
PCMH recognition will help CHCs fill a crucial role in serving an expanded number of Medicaid-eligible and insured patients, better position CHCs to be eligible for enhanced reimbursement, and enable them to participate in Accountable Care Organizations (ACOs).
The PCMH Learning Collaborative:
The PCMH Learning Collaborative is the Virginia Community Healthcare Association’s coordinated approach to Patient Centered Medical Home transformation and recognition for Virginia’s Community Health Centers.
Through its Learning Collaborative, the Association offers tools, resources and support for Community Health Centers at all levels of PCMH readiness. This includes one on one coaching, webinars, teleconferences, documentation sharing, face to face meetings and other resources to enable all CHCs in Virginia to be successful in their goal of achieving PCMH recognition.
